ABSTRACT

OBJETIVO: Este trabalho aborda sobre características referente aos exames citopatológicos do colo do útero em Altamira, coletado no Sistema de Informação do Câncer, dentro do período de 2014 a 2020. Observou-se também a qualidade da interpretação dos principais resultados encontrados, sobre a técnica de coleta e qualidade de exames. O objetivo é analisar o perfil epidemiológico dos exames citopatológicos do colo do útero do município. MÉTODO: A metodologia realizada foi estudo quantitativo, de corte transversal, epidemiológico, descritiva e analítico. RESULTADOS: Verificou- se um crescimento anual na taxa de cobertura do exame do preventivo no período de 2014 a 2019, que está ligado à implementação do Plano de Desenvolvimento Regional Sustentável do Xingu, e que esse crescimento mostra uma diferença estatística significativa entre a taxa de cobertura de Altamira, Pará, Brasil. Observou-se presença de falhas no preenchimento da ficha de notificação é referente ao campo da escolaridade das pacientes que não apresentam registro. Quanto a faixa etária mais frequente que realizam o exame do preventivo está entre 25 a 34 anos e as lesões intraepiteliais do colo uterino mais frequentes são: a de baixo grau que corresponde à população jovem (<34 anos) e de alto grau entre 25 a 44 anos.


OBJECTIVE: This paper deals with characteristics related to cytopathological examinations of the cervix in Altamira, collected in the Cancer Information System, within the period from 2014 to 2020. It was also observed the quality of interpretation of the main results found, on the technique collection and quality of exams. The objective is to analyze the epidemiological profile of cytopathological tests of the cervix in the city. METHOD: The methodology used was a quantitative, cross- sectional, epidemiological study, descriptive and analytical approach. RESULTS: As a result, there was an annual growth in the coverage rate of the preventive exam in the period from 2014 to 2019, which is linked to the implementation of the Xingu Sustainable Regional Development Plan, and that this growth shows a significant statistical difference between the coverage rate of Altamira, Pará, Brazil. It was observed the presence of failures in completing the notification form referring to the field of education of patients who do not have a record. As for the most frequent age group that undergoes the preventive examination, it is between 25 and 34 years old and the most frequent intraepithelial lesions of the uterine cervix are: low-grade, which corresponds to the young population (<34 years) and high-grade, between 25 and 34 years old. 44 years.


OBJETIVO: En este trabajo se abordan las características relacionadas con los exámenes citopatológicos de cérvix en Altamira, recogidos en el Sistema de Información del Cáncer, en el periodo comprendido entre 2014 y 2020. También se observó la calidad de interpretación de los principales resultados encontrados, sobre la técnica de recolección y calidad de los exámenes. El objetivo es analizar el perfil epidemiológico de los exámenes citopatológicos de cuello uterino en la ciudad. MÉTODO: La metodología utilizada fue un estudio cuantitativo, transversal, epidemiológico, de abordaje descriptivo y analítico. RESULTADOS: Como resultado, se observó un crecimiento anual de la tasa de cobertura del examen preventivo en el período de 2014 a 2019, que está vinculado a la implementación del Plan de Desarrollo Regional Sostenible Xingu, y que este crecimiento muestra una diferencia estadística significativa entre la tasa de cobertura de Altamira, Pará, Brasil. Se observó la presencia de fallas en el llenado del formulario de notificación referente al campo de la educación de los pacientes que no tienen un registro. En cuanto al grupo de edad más frecuente que se somete al examen preventivo, es entre 25 y 34 años y las lesiones intraepiteliales del cuello uterino más frecuentes son: de bajo grado, que corresponde a la población joven (<34 años) y de alto grado, entre 25 y 44 años.

Objetivo: compreender vivências e sentidos atribuídos pelas mulheres ao exame Papanicolau e ao cuidado de Enfermagem. Métodos: um estudo com abordagem qualitativa do tipo retrospectivo realizado entre os meses de outubro e novembro de 2019 na Atenção Primária à Saúde de Fortaleza, Ceará. Foram entrevistadas 24 mulheres, tendo como critérios de inclusão ter idade igual ou superior a 18 anos, usuárias da Unidade Atenção Primária em Saúde há pelo menos um ano e que tenham realizado prevenção anteriormente com a(o) enfermeira(o). Os dados foram coletados pela entrevista semiestruturada tiveram seus conteúdos analisados em categorias. Resultados: com base nos dados, foi possível perceber que as mulheres tinham idade de 20 a 66 anos, ensino fundamental à pós-graduação. Realizaram o exame há um ano. Buscaram-no para prevenção de doenças e atendimento a queixas. Desconheceram a relação com prevenção do câncer de colo uterino. Destacaram a invasão do exame ao corpo. Relataram ansiedade, nenhum esclarecimento, dificuldades de vínculo, conforto e segurança. Para elas a Enfermagem não possui competência para realização do exame. Conclusão:o estudo possibilitou o conhecimento dos motivos e sentimentos sobre o exame. Recomendam-se estudos avaliativos na Atenção Básica que produzam evidências necessárias à melhoria da gestão do cuidado à mulher.(AU)

They sought him for disease prevention and complaints. They did not know the relationship with the prevention of cervical cancer. They highlighted the invasion of the body by examination. They reported anxiety, no clarification, bonding difficulties, comfort and security. For them, Nursing does not have the competence to carry out the exam. Conclusion: the study enabled the knowledge of the reasons and feelings about the exam. Evaluative studies are recommended in Primary Care that produce evidence needed to improve the management of care for women.(AU)

Resultados: con base en los datos, fue posible notar que las mujeres tenían entre 20 y 66 años, desde la enseñanza básica hasta la posgrado. Hicieron el examen hace un año. Lo buscaban para prevención de enfermedades y denuncias. Desconocían la relación con la prevención del cáncer de cuello uterino. Destacaron la invasión del cuerpo por examen. Refirieron ansiedad, falta de aclaración, dificultades de vinculación, comodidad y seguridad. Para ellos, Enfermería no tiene competencia para realizar el examen. Conclusión: el estudio permitió conocer los motivos y sentimientos sobre el examen. Se recomiendan estudios evaluativos en Atención Primaria que produzcan las evidencias necesarias para mejorar la gestión del cuidado a la mujer.(AU)

ABSTRACT

Background & objectives: Human papillomavirus (HPV) infection is known to be the main cause of cervical cancer. This study aimed to determine the prevalence of high-risk HPV genotypes in smear specimens taken from women who had normal or abnormal cytology using a multiplex PCR method. Methods: The study included 270 women aged between 19 and 69 yr with or without suspicious cervical abnormalities. A Pap smear sample from each patient was cytologically examined, and HPV typing was performed using a multiplex fluorescent PCR method. Those who were high-risk HPV positive and had a normal or abnormal cytology were further evaluated by colposcopy and biopsy. Results: The total HPV positivity was 43 per cent (116/270). HPV positivity in the patients with an abnormal cytology was 77 per cent (33/43), whereas it was only 37 per cent (83/227) in women with normal cytology, which showed a significant difference (P<0.05). HPV positivity was also related to the age group when all the subjects were considered (P<0.05), and the highest prevalence of HPV infection was in the 30-39 yr age group. High-risk HPV types 16, 18, 31, 35, 51 and 56 were more common in the normal cytology patients, whereas high-risk HPV types 16, 31, 35, 45, 58 and 68 were commonly found in the abnormal cytology patients. Interpretation & conclusions: The determination of high-risk HPV genotypes in women with clinically suspicious cervical lesions should be conducted during an annual follow-up, irrespective of a normal or abnormal cytology by the age of 30 years or above.

@#Introduction: The greatest protection against cervical cancer can be achieved through the combination of cervical screening via repeated Pap tests and HPV vaccination before first sexual intercourse. This study aimed to explore women’s perceptions and experiences of Pap test and colposcopy examination to avoid advance stage of cervical cancer. Methods: An exploratory qualitative approach was carried out using face-to face semi-structured interviews with 22 women attending for their Pap test appointment or colposcopy examination in the main hospital in Kirkuk, Iraq. The study took place between October 2019 and March 2020. The collected data were analysed using inductive content analysis. Results: The findings revealed gap in knowledge regarding Pap test and colposcopy investigation among participants. Overall, three main categories emerged during data analyses namely, women’s awareness of Pap test; challenges to access cervical screening and colposcopy services; women’s views and suggestions to improve their access to cervical screening cervices. Conclusion: Currently, women living in Iraq have no access to regular cervical screening due to unavailability of the cervical screening programme in Iraq. This study provides evidence to develop strategies to enhance the existing cervical cancer prevention services. Health policy makers should consider the establishment of regular population based cervical screening. Health promotion efforts and interventions should focus on challenges and barriers influence the screening behavior among women living in Iraq

Objetivo: apreender as percepções de educadoras sobre o Papanicolau associadas às questões de corpo. Método: qualitativo, exploratório e descritivo realizado com 18 educadoras de uma escola pública em Petrolina-PE, após aprovação pelo Comitê de Ética, parecer nº 4.048.369. Empregou-se o recurso gráfico, juntamente, à entrevista semi-estruturada, tratados pela análise semiológica de imagem e pela análise temática de conteúdo. Resultados: as educadoras conhecem a importância do exame, mas apresentaram superficialidade sobre o objetivo e materiais utilizados, além de sentimentos negativos como constrangimento, dor, desconforto, dentre outros. Logo, o profissional de saúde precisa considerar as especificidades femininas para a escolha do material, assim como, valorizar o que cada corpo pode sentir e falar, independente das queixas. Conclusão: assim, sob uma perspectiva reducionista percebe-se que um corpo que fala é silenciado pelas dimensões de descuido e não de cuidado em saúde


Objective: to apprehend the perceptions of educators about Pap smears associated with body issues. Method: qualitative, exploratory and descriptive carried out with 18 educators from a public school in Petrolina-PE, after approval by the Ethics Committee, opinion nº 4.048.369. The graphic resource was used, together with the semi-structured interview, treated by the semiological image analysis and the thematic content analysis. Results: the educators know the importance of the exam, but they were superficial about the purpose and materials used, in addition to negative feelings such as embarrassment, pain, discomfort, among others. Therefore, the health professional needs to consider women's specificities when choosing the material, as well as valuing what each body can feel and say, regardless of the complaints. Conclusion: thus, from a reductionist perspective, it is perceived that a speaking body is silenced by the dimensions of carelessness and not health care


Objetivo: comprender las percepciones de los educadores sobre las pruebas de Papanicolaou asociadas a problemas corporales. Método: cualitativo, exploratorio y descriptivo realizado con 18 educadores de un colegio público de Petrolina-PE, previa aprobación del Comité de Ética, dictamen nº 4.048.369. Se utilizó el recurso gráfico, junto con la entrevista semiestructurada, tratado por el análisis de imagen semiológico y el análisis de contenido temático. Resultados: los educadores conocen la importancia del examen, pero fueron superficiales sobre el propósito y los materiales utilizados, además de sentimientos negativos como vergüenza, dolor, malestar, entre otros. Por tanto, el profesional de la salud debe tener en cuenta las especificidades de las mujeres a la hora de elegir el material, así como valorar lo que cada cuerpo puede sentir y decir, independientemente de las quejas. Conclusión: así, desde una perspectiva reduccionista, se percibe que un cuerpo hablante es silenciado por las dimensiones del descuido y no del cuidado de la salud

Resumen Introducción: El cáncer de cérvix es el segundo en frecuencia y el tercero en mortalidad; la infección por el virus del papiloma humano (VPH) está asociada al riesgo de cáncer; sin embargo, no se conoce el riesgo acumulado a 1, 2, 3, 5 y 10 años. Objetivo: Determinar el riesgo de las pacientes VPH positivo con evaluación inicial citológica negativa para desarrollar lesión intraepitelial de alto grado a lo largo del tiempo. Método: Se realizó una revisión sistemática de artículos en inglés y español de los últimos 20 años, usando las bases de datos PubMed, Cochrane, LILACS, ProQuest y Embase. Se incluyeron ensayos clínicos aleatorizados en los que se determinaba el estado VPH y se realizaba seguimiento con citología cervicovaginal a 1, 2, 3, 5 y 10 años en mujeres de 20-64 años. Resultados: Se incluyeron siete ensayos clínicos aleatorizados, con un total de 98.521 mujeres, de ellas 8820 VPH positivo y 89.701 VPH negativo al ingreso, seguidas hasta por 10 años con citología cervicovaginal, encontrando que la infección por VPH es un factor de riesgo para desarrollar lesiones intraepiteliales de alto grado a 2, 5 y 10 años, con un riesgo relativo de 110.94 (79.41-154.97), 83.65 (55.22-126.73) y 29.71 (5.72-154.33), respectivamente. Conclusiones: La infección por VPH es un factor de riesgo importante para el desarrollo de lesiones intraepiteliales de alto grado a 2, 5 y 10 años.


Abstract Introduction: Cervical cancer is the second in frequency and the third in mortality, infection by the human papillomavirus (HPV) is associated with the risk of increased cancer; however, the cumulative risk of 1, 2, 3, 5 and 10 years is not known. Objective: To determine the risk of HPV-positive patients with negative initial cytological evaluation for developing high-grade intraepithelial lesion over time. Method: A systematic review of articles in English and Spanish in the last 20 years was carried out, using the PubMed, Cochrane, LILACS, ProQuest and Embase databases. Randomized clinical trials were included in which HPV was performed and subsequent follow-up with cervicovaginal cytology at 1, 2, 3, 5 and 10 years in women aged 20-64 years. Results: Seven randomized clinical trials were included, a total of 98,521 women, 8820 with positive HPV and 89701 negative on admission and followed up for up to 10 years with cervicovaginal cytology. Finding that HPV infection is a risk factor for developing high-grade intraepithelial lesion at 2, 5 and 10 years with a relative risk of 110.94 (79.41-154.97), 83.65 (55.22- 126.73) and 29.71 (5.72-154.33), respectively. Conclusions: HPV infection is an important risk factor for the development of high-grade intraepithelial lesion at 2, 5 and 10 years.

Objective: Squamous cell carcinoma of the anus (SCCA) is associated with human papillomavirus (HPV) infection in almost 90% of the cases. Its incidence is alarmingly high among men who have sex with men (MSM) and continues to increase at an average rate of 2% per year. The objective of the present study is to evaluate the usefulness and performance of liquid-based anal cytology as a screening tool for prevention and early detection of SCCA in a cohort of at-risk men. Method: We conducted a retrospective study including 111 MSM, aged between 22 and 62 years old, who underwent anal cytological screening with a liquid-based Pap test at our sexually transmitted diseases (STDs) clinic from January 2015 to March 2017. Results: Out of 111 anal smears, 57 (51,4%) resulted negative, 42 (37,8%) abnormal, and 12 (10,8%) unsatisfactory for the cytological evaluation. Only patients with an abnormal cytology underwent anoscopy and subsequent biopsy. The histological results were as follows: negative for squamous intraepithelial lesion (SIL) in 5 cases, low-grade SIL (L-SIL) in 21, high-grade SIL (H-SIL) in 5, SCCA in 1. Five patients had a normal anoscopy and biopsy was not taken. Conclusion: Liquid-based cytology, reducing the "darkening factors" typical for the conventional smears, has a higher positive predictive value than the traditional technique. Moreover, a cytological diagnosis of atypical squamous cells of undetermined significance (ASC-US) or L-SILmay hide a severe dysplasia or even a carcinoma. Thus, all patients with an abnormal anal cytology at any grade should be considered for anoscopy. (AU)

Background and Objectives: reproductive tract infections are considered an important demand for women's health, due to their high prevalence in the population and the consequences they can cause, such as premature birth, infertility and cervical cancer. There are still women who do not undergo cytopathological examination, capable of preventing these infections, either due to lack of guidance or opportunity, such as quilombola women. This study aimed to identify factors associated with cervicovaginal infections in quilombola women from Feira de Santana, Bahia. Methods: a descriptive study, carried out in a transversal way and with a quantitative approach. Data collection was carried out from November 2019 to January 2020, with the application of an anamnesis form prepared by the team, with variables used in other studies, such as risk factors. Bivariate analysis was performed to obtain prevalence ratios (PR) between infections and the characteristics found. Results: a total of 82 women were studied, with an average age of 45.3 years. The main vaginal infections were caused by Gardnerella vaginalis (17.1%), Trichomonas vaginalis (8.5%), Cocci (8.5%), Candida spp. (6.2%) and Fusobacterium spp. (1.2%). In 88.2% of infected women, inflammation was present. Conclusion: the results obtained in this study showed a correlation between the inflammatory process in women and the presence of some infection. With regard to infections, there was a higher prevalence in women with low education, without a steady partner and who did not undergo regular preventive examination.(AU)


Justificativa e Objetivos: as infecções do trato reprodutivo são consideradas uma demanda importante para a saúde da mulher, devido à sua alta prevalência na população e às consequências que podem causar, como parto prematuro, infertilidade e câncer do colo do útero. Ainda há mulheres que não realizam exame citopatológico, capaz de prevenir essas infecções, seja por falta de orientação ou oportunidade, como as mulheres quilombolas. Este estudo teve como objetivo identificar fatores associados a infecções cervicovaginais em mulheres quilombolas de Feira de Santana, Bahia. Métodos: estudo descritivo, realizado de forma transversal e com abordagem quantitativa. A coleta de dados foi realizada no período de novembro de 2019 a janeiro de 2020, com a aplicação de uma ficha de anamnese elaborada pela equipe, com variáveis utilizadas em outros estudos, como fatores de risco. A análise bivariada foi realizada para obter razões de prevalência (RP) entre as infecções e as características encontradas. Resultados: foram estudadas 82 mulheres, com média de idade de 45,3 anos. As principais infecções vaginais foram causadas por Gardnerella vaginalis (17,1%), Trichomonas vaginalis (8,5%), Cocci (8,5%), Candida spp. (6,2%) e Fusobacterium spp. (1,2%). Em 88,2% das mulheres infectadas, a inflamação estava presente. Conclusão: os resultados obtidos neste estudo mostraram uma correlação entre o processo inflamatório em mulheres e a presença de alguma infecção. No que se refere às infecções, houve maior prevalência em mulheres com baixa escolaridade, sem companheiro fixo e que não realizavam exame preventivo regular.(AU)


Justificación y Objetivos: las infecciones del aparato reproductor son consideradas una demanda importante para la salud de la mujer, por su alta prevalencia en la población y las consecuencias que pueden ocasionar, como parto prematuro, infertilidad y cáncer de cuello uterino. Todavía hay mujeres que no se hacen un examen citopatológico, capaz de prevenir estas infecciones, ya sea por falta de orientación o de oportunidad, como las mujeres quilombolas. Este estudio tuvo como objetivo identificar los factores asociados a las infecciones cervicovaginales en mujeres quilombolas de Feira de Santana, Bahia. Métodos: estudio descriptivo, realizado de forma transversal y con enfoque cuantitativo. La recolección de datos se realizó de noviembre de 2019 a enero de 2020, con la aplicación de un formulario de anamnesis elaborado por el equipo, con variables utilizadas en otros estudios, como los factores de riesgo. Se realizó análisis bivariado para obtener razones de prevalencia (RP) entre las infecciones y las características encontradas. Resultados: se estudiaron un total de 82 mujeres, con una edad promedio de 45,3 años. Las principales infecciones vaginales fueron causadas por Gardnerella vaginalis (17,1%), Trichomonas vaginalis (8,5%), Cocci (8,5%), Candida spp. (6,2%) y Fusobacterium spp. (1,2%). En el 88,2% de las mujeres infectadas hubo inflamación. Conclusión: los resultados obtenidos en este estudio mostraron una correlación entre el proceso inflamatorio en la mujer y la presencia de alguna infección. En cuanto a las infecciones, hubo mayor prevalencia en mujeres con baja escolaridad, sin pareja estable y que no realizaban examen preventivo periódico.(AU)

ABSTRACT Objective: to analyze factors associated with the monitoring of actions to control cervical cancer in the Family Health Strategy, in a health region of northeastern Brazil. Method: a cross-sectional study conducted from January to March 2019 by means of interviews with 241 physicians and nurses from the Family Health Teams of the Vitória da Conquista health region, Bahia, Brazil. Adequate monitoring was assessed by the degree of actions taken to promote, prevent and actively seek to control cervical cancer. Three blocks of variables were tested as explanatory: professional characterization and training; organization of the unit and access to cytopathological exam; and care coordination and integration. Poisson regression with robust variance was employed, adopting hierarchical entry of variables. Results: 51.9% (95% CI: 45.5-58.2) of the professionals performed adequate monitoring for the control of cervical cancer. Being a nurse, working in the municipality's primary care network (≥2 years), disclosure of results collection through posters and other communication means, existence of a high-grade lesion, time to perform the biopsy ≤1 month, and agility in the release of the reports were elements associated to the outcome. Conclusion: even with the extended coverage of the Family Health Strategy, small municipalities in the Northeast have characteristics that impose obstacles to comprehensiveness, favoring the incidence of high-grade lesions and greater difficulty in the control of cervical cancer. Assessing the assistance quality in this level revealed challenges in the regionalized network.


RESUMEN Objetivo: analizar factores asociados al monitoreo de las acciones para el control del cáncer cervical en la Estrategia de Salud de la Familia, en una región de salud del noreste de Brasil. Método: estudio transversal realizado entre enero y marzo de 2019, por medio de entrevistas con 241 médicos y enfermeros de los Equipos de Salud de la Familia de la región de salud de Vitória da Conquista, Bahía, Brasil. El monitoreo adecuado se evaluó por medio del grado en el que se realizaron acciones de promoción, prevención y búsqueda activa para el control del cáncer cervical. Se probaron tres bloques de variables como explicativos: caracterización y capacitación profesional; organización de la unidad y acceso al examen citopatológico; y coordinación de la atención e integración asistencial. Se usó regresión de Poisson con varianza robusta, adoptando entrada jerárquica de variables. Resultados: 51,9% (IC 95%: 45,5-58,2) de los profesionales realizaban un monitoreo adecuado para el control del cáncer cervical. Ser enfermero, desempeñarse en el sector de atención primaria del municipio (≥2 anos), la exposición de los resultados de los exámenes a través de posters y otros medios de comunicación, la existencia de lesiones de alto grado, el tiempo para realizar la biopsia ≤1 mes, y la agilidad en la emisión de los informes fueron elementos asociados al resultado. Conclusión: incluso con extensa cobertura de la Estrategia de Salud de la Familia, los municipios pequeños del noreste acumulan características que imponen obstáculos a la integralidad, lo que favorece la incidencia de lesiones de alto grado y mayor dificultad para controlar el cáncer cervical. Evaluar la calidad de la asistencia en este nivel dejó al descubierto desafíos en la red regionalizada.

Ser enfermeiro, atuar na atenção primária do município (≥2 anos), divulgação da coleta por cartazes e outros veículos de comunicação, existência de lesão de alto grau, tempo de realização da biópsia ≤1 mês e agilidade na liberação dos laudos foram elementos associados ao desfecho. Conclusão: mesmo com alta cobertura da Estratégia Saúde da Família, municípios de pequeno porte do Nordeste acumulam características que conferem obstáculos à integralidade, favorecendo a incidência de lesão de alto grau e maior dificuldade de controle do câncer cervicouterino. Avaliar a qualidade da assistência nesse nível revelou desafios em rede regionalizada.

@#Introduction: Cervical cancer is responsible for women’s morbidity and mortality throughout the world. In developing countries women usually present in advanced stage, awareness of cervical cancer can enhance women inclination towards regular cervical screening which can detect early pre-invasive lesions. The aim of the study was to assess the awareness of cervical cancer among King Faisal University community visiting gynecology clinic, Al Ahsa Saudi Arabia. Methods: A questionnaire oriented cross-sectional study was conducted and 385 females were enrolled by systematic random sampling. The questionnaire contained 14 questions about cervical cancer and six barrier statements for cervical screening. SPSS software version 26.0 was used for data entry and analysis. Results: Total 385 females participated in the study. Majority 73.2% (n=282) had heard about cervical cancer and found significant association with age 36-45yr (p=0.001), married with two children (p=0.003), employed (p=0.001), Master level education (p=0.001) and with Saudi nationality (p=0.028). Logistic regression revealed age increases likely hood of adequate knowledge almost by one fold (odds ratio 0.954, 95% CI: 0.925-0.984) and having Saudi nationality increases it by two folds (odds ratio 2.056, 95%CI: 1.228-3.440). Almost 80% were aware of screening test, 18.4% had undergone one time an opportunistic screening test. Significant proportion of Saudi nationals had heard of HPV vaccines (p= 0.04). Conclusion: Participants had adequate knowledge of cervical cancer but had low screening test practice. There is need to implement regular cervical screening practice at national level.

ABSTRACT

Objective: explore the factors associated with knowledge about the Pap test in female users of the Municipal Public Health Service. Method: quantitative, cross-sectional study, conducted in the state of São Paulo with 180 women. A tool was used to characterize profiles and determine knowledge. A descriptive statistical analysis, chi-square test for bivariate analysis and binary logistic regression for multivariate analysis were performed. Results:the women were +/-43.6 years old, most worked at paid jobs, were brown in skin color, had a partner, did not use condoms, did not have any gynecological complaints and were tested regularly. Knowledge was satisfactory in regard to the location where and how the test is done and unsatisfactory in terms of target population and frequency. Conclusion:the variables associated with satisfactory knowledge were white skin color, monthly household income, gynecological complaints and regular testing. W omen with a monthly household income of two or more minimum wages were more likely to have adequate knowledge about the Pap test.


Objetivo: conhecer os fatores que se associam ao conhecimento sobre o exame de Papanicolaou em mulheres usuárias do Serviço Público Municipal de Saúde. Método: estudo quantitativo e transversal, realizado no interior de São Paulo com 180 mulheres. Utilizou-se instrumento para caracterizar o perfil e verificar o conhecimento. Realizou-se análise estatística descritiva, teste qui-quadrado para análise bivariada e regressão logística binária para multivariada. Resultados:as mulheres possuíam +/-43,6 anos, a maior parte tinha trabalho remunerado, cor parda, companheiro, não utilizava preservativo, não apresentava queixas ginecológicas e realizava o exame em tempo adequado. O conhecimento foi satisfatório quanto ao local e à forma de realização do exame e insatisfatório quanto à população-alvo e periodicidade. Conclusão:associaram-se ao conhecimento satisfatório as variáveis cor da pele branca, renda familiar, queixa ginecológica e realizar o exame em tempo adequado; mulheres com renda familiar mensal de dois ou mais salários mínimos apresentaram mais chances de ter conhecimento adequado sobre o exame de Papanicolaou.

ABSTRACT

Background: Carcinoma of the uterine cervix is a major health problem faced by the Indian women. Regular cervical cytological examination by all sexually active women can prevent the occurrence of carcinoma cervix. Early detection of cervical cancer is possible with Pap smear tests. Methods: Women above 25 years of age, living in study area and want to participate in study were included. Total 2352 women were enrolled in study. House to house visits were conducted in all the village area by using simple random sampling method. Information about cervical cancer was given. Pap test for cervical cancer screening was carried out by gynaecologist. Cytological examination and confirmation was done by pathologists.Results: A total of 3001 women had attended village level IEC session and out that, 2352 (78.4%) women took part in the screening program. Out of these 2352 women, 2007 women (85.3% compliance) had given consent for physical cervical examination and Pap smear. The incidence of cervical cancer was 0.2% on the basis of clinical examination and biopsy.Conclusions: Higher compliance for undergoing vaginal examination and Pap test shows the positive health seeking behaviour of the women but for that, strong IEC and sensitization about the disease must be done to improve the participation. Sensitivity of Pap test was poor and couldn’t find true positive cases.

ABSTRACT

Background: India accounts for 25% of total deaths from cervical cancer. Screening for cervical cancer reduces mortality by early detection and treatment. So, a study was conducted among rural women of Ganjam district with the objective to know knowledge, attitude and practice of cervical screening among them.Methods: A community based cross sectional study was conducted from January to February 2019 among ever married women of more than 30 years of age residing in villages of Chhatrapur block of Ganjam district. Sample size was calculated to be 280 by formula Z2pq/l2. Multistage random sampling and PPS method were used to select participants. A semi-structured questionnaire was used to collect data and all data were analysed in SPSS.Results: Among 280 participants, though 70.7% had heard about cervical cancer, only 32.5% knew that there are tests for screening. Only 13.2% of participants said that Pap smear test was to be taken regularly. However, majority didn’t know who should undergo screening test and what age tests should be started. Main source of information for them was health worker (39%). Only 14.6% had any type of cervical screening in their lifetime. The main reason for not going for screening was ignorance (53.9%) followed by thinking tests are not needed unless any symptoms appearedConclusions: Proportion of participants with proper knowledge and awareness regarding cervical cancer screening was low which led to poor uptake of screening. So, awareness regarding cervical cancer screening should be increased. Health camps and education programs should be conducted regularly.

ABSTRACT Objective: To analyze women's perception of the quality of the service provided for colposcopy test in the city of Belém (PA), Brazil. Method: Quantitative-descriptive study on 400 women who underwent Pap test on the public healthcare network, using the Service Quality (SERVQUAL) model. We used the Cronbach's alpha index to measure the reliability of the scale, and data were analyzed by quartiles of the gaps of the dimensions: tangibility, responsiveness, reliability, assurance, and empathy. Results: The assurance dimension, corresponding to the knowledge and courtesy of staff, featured the highest degree of importance to users; and empathy, which corresponds to the staff's concern with women's needs, featured the lower degree. The most expressive negative gaps concerned the structure of services and the attitude of healthcare professionals when collecting the material. Conclusion: All gaps featured negative relationship between what was expected and perceived, expressing dissatisfaction regarding the service.


RESUMEN Objetivo: Analizar la percepción de mujeres acerca de la calidad del servicio de colpocitología oncótica en Belém (PA). Método: Estudio cuantitativo-descriptivo con 400 mujeres que realizaron el examen de colpocitología oncótica en la red pública de salud, utilizando el modelo Service Quality (SERVQUAL). Se utilizó el Alpha de Cronbach para medir la confiabilidad de la escala, y los datos fueron analizados por los cuartiles de los gaps de las dimensiones: tangibilidad, responsividad, confiabilidad, credibilidad y empatía. Resultados: La dimensión credibilidad, correspondiente al conocimiento y a la cortesía de los funcionarios, presentó mayor grado de importancia para las usuarias; y la empatía, que corresponde a la preocupación demostrada por los funcionarios con las necesidades de las mujeres, exhibió menor grado. Los gaps negativos más expresivos se refieren a la estructura de los servicios y a la actitud de los profesionales con ocasión de la recolección del material. Conclusión: Todos los gaps presentaron relación negativa entre lo esperado y lo percibido expresando la insatisfacción con relación al servicio.

ABSTRACT

El objetivo fue determinar variables socio-sanitarias asociadas al uso adecuado de la citología cervico-uterina en las mujeres del área rural-Cartagena. Estudio analítico de corte transversal. Participaron 638 mujeres de 25 años con vida sexual activa o entre los 25 a 69 años que iniciaron o no su vida sexual del área rural-Cartagena. Se realizó un muestreo por conglomerados conformado por 15 corregimientos de la ciudad de Cartagena. La información recolectada fue incluida en una hoja de cálculo de Microsoft Excel y analizada mediante el programa SPSS versión 20. Se encontró que un 82,1% (524) de las mujeres se realizan la citología, pero solo el 35% (223) cumple con el esquema 1-1-3, se refleja mayor cumplimiento en aquellas mujeres cuya EPS fue la responsable del pago de dicho servicio. Las mujeres del área del área rural de Cartagena se han realizado la citología cervico uterina en algún momento de su vida sin embargo más de la mitad no cumplen con el esquema 1-1-3, la variable socio-sanitaria que se encuentra asociada estadísticamente con un mayor cumplimiento de esquema, es el no tener que pagar por este servicio, lo que influye positivamente en un uso adecuado de la Citología.


The Objective was to determine socio-sanitary variables associated to the adequate use of cervico-uterine cytology in rural women-Cartagena. Analytical cross-sectional study. There were 638 women of 25 years of age with active sexual life or between 25 and 69 years old who started or not their sexual life in the rural area-Cartagena. A cluster sampling conformed by 15 corregimientos of the city of Cartagena was carried out. The information collected was included in a Microsoft Excel spreadsheet and analyzed using the SPSS version 20 program. It was found that 82.1% (524) of women underwent cytology, but only 35% (223) complies with the 1-1-3 scheme, a greater compliance of this is reflected in those women whose EPS was responsible for the payment of said service. Women from the rural area of Cartagena have undergone vaginal cytology at some point in their lives, however more than half do not comply with the 1-1-3 scheme, the socio-health variable that is statistically associated with a greater compliance with the scheme is not having to pay for this service, which positively influences the proper use of Cytology.

ABSTRACT

OBJECTIVE: Human papillomavirus testing by self-sampling and urine sampling might be alternatives to Papanicolaou test (Pap test) for cervical cancer screening (CCS), and may increase compliance and adherence thereto. The present study aimed to explore satisfaction and preferences for cervical screening modalities among Korean women. METHODS: In total, 732 women aged between 20 and 69 years responded to a questionnaire designed to survey the women's perceived satisfaction for the 3 CCS modalities: clinician-collected Pap test, self-collected vaginal sampling (self-sampling) and urine sampling. RESULTS: Overall satisfaction was significantly higher with both the self-sampling and urine sampling than the clinician-collected Pap test (odds ratio [OR]=2.01; 95% confidence interval [CI]=1.48–3.00 and OR=2.47; 95% CI=1.75–3.48, respectively). Psychological distress, including embarrassment, pain, anxiety, discomfort, and stress, with self-sampling and urine sampling were significantly lower than that with the Pap test. 52% of participants reported preferences for self-sampling in the next screening round. CONCLUSIONS: Korean women were more likely to report satisfaction with alternative modalities (self-sampling and urine sampling) for CCS in comparison to the Pap test. This suggests that self-collected modalities may help with improving CCS uptake rates by eliminating burden related with the Pap test. However, further studies for test accuracy and cost-effective analysis of the alternative modalities should be conducted in order to apply CCS.

Resumo Introdução O câncer cervical é uma neoplasia maligna que atinge milhares de mulheres, e a estratégia de prevenção considerada mais efetiva para essa doença é por meio Papanicolaou. Objetivo: o objetivo deste artigo é compreender os sentidos do Papanicolaou para um grupo de mulheres que realizou a prevenção do câncer cervical. Método Trata-se de uma pesquisa qualitativa, com abordagem etnográfica, com nove mulheres em idade reprodutiva, moradoras do Jardim Keralux, localizado na zona leste da cidade de São Paulo. A escolha desse local deveu-se ao fato de que é um bairro cuja população é considerada vulnerável para o acesso das redes de prevenção ao câncer de colo do útero. Foi realizada uma análise de conteúdo das entrevistas. Resultados O Papanicolaou é carregado de significados que deslizam entre os planos físico e moral da vida desse grupo de mulheres. O Papanicolaou torna visível não só a doença no interior do corpo feminino, reforçando a crença nas possibilidades de tratamento e cura, mas também o leque de relações nas quais as mulheres estão envolvidas, particularmente as de gênero do contexto sociocultural. Conclusão O Papanicolaou passa a fazer parte do cuidado à saúde desse grupo de mulheres, reforçando suas crenças na prevenção do câncer cervical.


Abstract Brackground Cervical cancer is a malignant neoplasm that affects thousands of women, and the Papanicolaou (Pap smear) test is considered the most effective prevention strategy. Objective: the purpose of this study is to understand the significance of the Pap test for a group of women undergoing cervical cancer prevention. Method A qualitative study of ethnographic approach was conducted with nine women at reproductive age from the neighborhood of Jardim Keralux, located in the eastern district of the city of Sao Paulo. The choice of this location was due to the fact that it is a neighborhood whose population is considered vulnerable for access to the cervical cancer prevention network. Data from the interviews were assessed by content analysis. Results The Papanicolaou test is permeated with meanings that range between the physical and moral planes of the lives of the women in this group. The Pap test makes disease visible in the female body, reinforcing belief in the possibilities of treatment and cure. It also enables visibility of an array of relationships in which women are involved, particularly regarding gender relations of the sociocultural context. Conclusion The Papanicolaou test becomes part of the health care of this group of women, reinforcing their beliefs in the prevention of cervical cancer.

RESUMEN Antecedentes: Un algoritmo puede definirse como un conjunto de operaciones y procedimientos que se deben seguir con el fin de resolver un problema. Objetivo: Conocer el cumplimiento del flujo de decisiones clínicas del protocolo preconizado en los algoritmos de derivación y de confirmación diagnóstica para la citología cervical atípica de significado indeterminado - H1, estipulados en el Programa Nacional de Búsqueda y Control del Cáncer Cervical de Chile. Métodos: Estudio epidemiológico de tipo descriptivo, observacional, cuantitativo y de seguimiento de una cohorte de mujeres portadoras de un primer informe de citología cervical atípica de significado indeterminado - H1, pertenecientes a los consultorios de Atención Primaria de Salud del área Metropolitana Sur de Santiago, Chile. El seguimiento fue a través de la aplicación de los "Algoritmo de derivación a especialista o U.P.C. del primer PAP atípico según la clasificación Bethesda 2001" y "Algoritmos para confirmación diagnóstica". Resultados: Se seleccionaron 446 mujeres con un primer informe H1. El cumplimiento de conductas de acuerdo al algoritmo de derivación fue de 42.4% y las conductas alternativas fue de 5.8%. La adherencia de conducta en los algoritmos para confirmación diagnóstica fue cumplida en un 89.6%. Conclusiones: El cumplimiento de las conductas ajustadas a algoritmo de derivación del primer Pap atípico fue realizado por la mitad de las mujeres y el cumplimiento del algoritmo de confirmación diagnóstica fue realizado por la gran mayoría.


SUMMARY Background: An algorithm can be defined as a set of operations and procedures that must be followed in order to solve a problem. Objective: Determine the compliance of the clinical decisions flow and procedures recommended by the ASCUS results derivation and diagnostic confirmation algorithms established in the National Program of Search and Control of Cervical Cancer of Chile. Methods: An epidemiological, descriptive, observational and quantitative study of a follow up investigation of a cohort of women with ASCUS cytological reports. These women belonging in the health care centers in the South Metropolitan area of Santiago de Chile. The algorithms "The Specialist Derivation Algorithm or UPC for the first atypical Pap according to the Bethesda 2001 classification" and "Diagnosis Confirmation Algorithm" were used for the follow-up process of these patients. Results: Women with a first ASCUS report were selected (n = 446). The compliance of the clinical decisions flow to derivation algorithm was 42.4% and alternative behaviors were 5.8%. In the diagnosis confirmation algorithm was 89.6% the compliance the recommended procedures. Conclusion: Half ASCUS carrier women followed the procedures recommended by the derivation algorithm. In the algorithms of diagnostic confirmation it is met mostly.

ABSTRACT

O câncer cervical é o quarto tipo de tumor mais frequente entre mulheres e quarta causa de mortalidade feminina por câncer no Brasil. Neste trabalho o objetivo foi demostrar o perfil das mulheres atendidas pelo SUS que apresentaram lesões intraepiteliais de baixo grau (LSIL) e alto grau (HSIL) no exame preventivo citopatológico realizado por um laboratório de citopatologia credenciado ao SUS que atende Cascavel-PR e distritos. No período de maio de 2014 a maio de 2015 foram observados 395 exames citopatológicos com LSIL e 160 com HSIL, cuja faixa etária predominante foi dos 25 aos 64 anos (70,64%). Em média observou-se os seguintes resultados: a JEC esteve presente em 90,56% das amostras; o colo cervical apresentou-se normal em 77,31% das mulheres e 43,58% destas mulheres em estudo haviam realizado um exame anterior em até um ano, sendo que o maior motivo para realização do mesmo foi o rastreamento (89,11%). Mais que 90% das mulheres não apresentaram sangramentos, seja após a relação sexual ou menopausa. As lesões também prevaleceram em mulheres atendidas na UBS (61,98%) mesmo elas sendo em menor número se comparadas àquelas atendidas por USF. Esses dados demostram a importância da realização desse exame, pois na maioria das lesões as mulheres não apresentaram sintomas ou alterações clínicas.


Cervical cancer is the fourth most frequent kind of cancer among women and the fourth cause of female mortality due to cancer in Brazil. The purpose of this work was to present the profile of women attended by the Brazilian Unified Health System (SUS) who showed low-grade (LSIL) and high-grade (HSIL) intraepithelial lesions at the cytopathology screening test performed by a SUS-accredited cytopathology laboratory serving the city of Cascavel-PR and its districts. In the period between May 2014 and May 2015, a total of 395 cytopathologic test presenting LSIL and 160 with HSIL were observed. The age group ranged from 25 to 64 years old (70.64%). The following average results were observed: squamocolumnar junction was present in 90.56% of the samples; normal cervix in 77.31% of women and 43.58% of the women in the study had already had an examination within the previous year, with the main reason for the present exam was screening (89.11%). More than 90% of women did not present any bleeding, either after sexual intercourse or menopause. The lesions also prevailed in women attending primary health care (61.98% of lesions) even though they were in lower number when compared to those served by family health programs. Such information demonstrates the importance of carrying out this examination, since most lesions are asymptomatic or do not show any clinical changes.
